How We Protect Personal Health Information

Ryding House is committed to protecting the privacy and confidentiality of the people who access our services. We collect, use, disclose, store and dispose of personal health information in accordance with Ontario's Personal Health Information Protection Act, 2004 (PHIPA) and other applicable legal and professional requirements.

Health Information Custodian

Ryding House Inc. is the health information custodian responsible for personal health information held by Ryding House.

How We Handle Personal Health Information

Ryding House collects only the information reasonably necessary to:

  • Assess eligibility and suitability for our services;
  • Plan, provide and coordinate care;
  • Support resident health, safety and continuity of care;
  • Communicate with authorized health care providers, referral partners and other individuals involved in a resident's care;
  • Administer our programs and services;
  • Meet legal, regulatory, professional and funding requirements; and
  • Improve the quality and safety of our services.

We generally obtain consent before collecting, using or disclosing personal health information, except where PHIPA or another law permits or requires us to act without consent.

Only staff and authorized service providers who require personal health information to perform their duties may access it. Ryding House uses administrative, physical and technological safeguards to protect information from loss, theft, unauthorized access, use, disclosure, copying, modification or disposal.

Ryding House does not sell personal health information.

Accessing Your Personal Health Information

You may request access to a record of your personal health information held by Ryding House. Requests may be made verbally or in writing to the Privacy Contact Person.

We may ask for information to confirm your identity and clarify which records you are requesting. Access will be provided within the timelines established by PHIPA, subject to any legal limitations or exceptions.

If we cannot provide access to all or part of a record, we will explain the reason and advise you of any available complaint or review process.

Requesting a Correction

If you believe that your personal health information is inaccurate or incomplete, you may ask Ryding House to correct the record.

Requests should identify the information you believe is inaccurate or incomplete and explain the correction being requested. Ryding House will review the request in accordance with PHIPA and notify you of the outcome.

We will review privacy concerns fairly and respond as promptly as possible. Raising a privacy concern or making a complaint will not affect your eligibility for or access to Ryding House services.
